Verification of Living Arrangements EAEDC

The verification of living arrangement is mandatory and can be made by:

Living Arrangement A shall be verified by:
- telephonic self-declaration.
Living Arrangement B shall be verified by:
- telephonic self-declaration.
Living Arrangement C shall be verified by:
- a statement from an authorized person at the halfway house, licensed chronic hospital, approved public medical institution, licensed intermediate care facility, public psychiatric institution or residential treatment center.
Living Arrangement D shall be verified by:
- telephonic self-declaration.
Living Arrangement E shall be verified by:
- a written statement from an authorized person at the rest home.
Living Arrangement F shall be verified by:
- a written statement from an authorized person at the therapeutic community center.
Living Arrangement H shall be verified by:
- telephonic self-declaration.
