Verification of Living Arrangements - EAEDC

The verification of living arrangement is mandatory and can be made by: 

  • Living Arrangement A shall be verified by:
    • telephonic self-declaration.
  • Living Arrangement B shall be verified by:
    • telephonic self-declaration.
  • Living Arrangement C shall be verified by:
    • a statement from an authorized person at the halfway house, licensed chronic hospital, approved public medical institution, licensed intermediate care facility, public psychiatric institution or residential treatment center.
  • Living Arrangement D shall be verified by:
    • telephonic self-declaration.
  • Living Arrangement E shall be verified by:
    • a written statement from an authorized person at the rest home.
  • Living Arrangement F shall be verified by:
    • a written statement from an authorized person at the therapeutic community center.
  • Living Arrangement H shall be verified by:
    • telephonic self-declaration.
