Complete Request (RFA)
At the Complete Request page, the Request for Assistance (RFA) will be disposed in one of three ways. Select one of three options from the Action drop-down box for each Request for Assistance:
- Print RFA will generate the form for your and the applicant’s signatures. This is the default selection for this page
Note: Signing the form establishes the application date for the requested program(s)
- Application Inquiry will cause the information to be stored in the database
- Terminate Request will cause the information to be deleted. If the applicant returns to file another application, there will be no record of this initial application
After a selection has been made, click Next. If you select Application Inquiry or Terminate Request, you must exit the interview.
- If the Print RFA (RA-1) option is selected, the Request for Assistance will appear on the Notice Preview page for review. Once it has been reviewed for accuracy, click the Print button
- to print the RFA, click print on the Print box. The Request for Assistance is printed and the Notice Preview page will still be displayed
- click X to close the Notice Preview page
- after closing the Notice Preview page, the Signature page will appear. You must check off that the RFA has been signed by the Department Representative
- if the client needs an EBT card, click the Request primary EBT card check box. Then follow the procedures described in the Electronic Benefits Transfer (EBT) book
- click Next to continue the application interview. The Household page will then appear
- click Finish if the case is being assigned to another case manager
Note: If the Request for Assistance Only workflow was selected, the workflow ends here. If the Request for Assistance workflow was selected, the option of Next or Finish is applicable. The Request for Assistance workflow continues through the complete SNAP application
Give a copy of the RFA to the applicant and file the original in the case record.
The Request for Assistance workflow is now completed, stored and unavailable for additional data entry from this workflow. If the applicant wants to apply for additional programs, a new Request for Assistance workflow must be completed.
