ADA Request for Modification
If, after implementing an Americans with Disabilities Act (ADA) accommodation, the applicant or client (hereafter referred to as client) requests modification of the accommodation, the modification request must be treated as a new accommodation request and submitted to the Client Assistance Coordinator for approval.
A new request and screening must be completed indicating the client is requesting a modification of the initial ADA accommodation. The procedures used for modifying an ADA accommodation are the same used for requesting an ADA accommodation.
